Through first-hand interviews, gripping footage, and expert analysis, the docuseries delves into the harrowing event in an attempt to shed light on the vagueness surrounding the crash. Flight 3054, arriving from Porto Alegre, failed to land safely, overrunning the runway and colliding with a nearby building. The catastrophic accident claimed the lives of all 187 passengers and crew on board, as well as 12 individuals on the ground, amounting to 199 total fatalities. The tragedy occurred during a period known as Brazil’s “aviation blackout,” characterized by systematic issues such as inadequate infrastructure, staffing shortages, and regulatory shortcomings within the country’s aviation sector. What Really Happened to Flight 3054?

On the evening of July 17, 2007, TAM Airlines Flight 3054 approached São Paulo’s Congonhas Airport under challenging weather conditions. The airport, situated in a densely populated urban area, featured short runways and lacked adequate safety zones, making landings particularly demanding. As an interviewee in the docuseries shares, flying into the airport feels as if one is taking a peek into people’s living rooms. As Flight 3054 attempted to land on the rain-slicked runway, it failed to decelerate properly. The aircraft skidded off the runway, crossed a busy avenue, and crashed into a TAM Express warehouse, resulting in a massive explosion.

All 187 passengers and crew members aboard lost their lives, along with 12 individuals on the ground. The crash site was engulfed in flames, complicating rescue efforts and leading to the complete destruction of the aircraft and significant damage to surrounding structures. The fire could only be contained after 4 days, with the authorities taking 7 long hours to release the list of passengers traveling on the flight.

What Caused the Crash of Flight 3054?

The investigation into Flight 3054’s crash revealed a confluence of human errors, mechanical issues, and infrastructural deficiencies. A critical factor was the misconfiguration of the aircraft’s thrust levers during landing. While the left engine’s thrust lever was set to idle, the right engine’s lever remained in the climb position, causing asymmetric thrust that prevented the aircraft from slowing down properly. This misalignment also inhibited the deployment of spoilers and thrust reversers, which are essential components for deceleration.

Compounding the problem was the runway’s condition. Recently resurfaced, the runway lacked the necessary grooves to channel water away, increasing the risk of hydroplaning. Despite previous reports from pilots about poor braking conditions, the runway remained operational. The lack of standardized procedures and inadequate response to known safety concerns contributed to the disaster. The combination of these factors – pilot error, mechanical limitations, and inadequate runway conditions – created a scenario that culminated in a fiery, deadly note. Many believe that if not for the negligence of multiple individuals, the outcome of Flight 3054’s journey would have been much better. Dario Scott, who lost his daughter in the crash, said:

“It’s very sad for us to relive what we went through, but I think that it is important for us to tell this story, so that people are aware and make sure that safety is always taken seriously.”

Who Was Held Responsible for Flight 3054’s Crash?

In the wake of the Flight 3054 disaster, Brazilian authorities launched extensive investigations to determine accountability. Initially, 11 individuals, including TAM Airlines executives and aviation officials, faced indictments on charges ranging from negligence to dereliction of duty. However, the case did not proceed in State Court and was transferred to the Federal Public Prosecutor’s Office. The investigation indicted the two pilots of Flight 3054, Kleyber Lima and Henrique Stefanini Di Sacco.

However, when formal charges were brought against the pilots, the prosecution chose to indict three other people instead: Marco Aurélio dos Santos de Miranda e Castro, TAM’s director of flight safety, Alberto Fajerman, the company’s vice president of operations, and Denise Abreu, the director of Anac. The prosecution argues that these individuals had failed to address known safety issues, such as the runway’s inadequate grooving and the decision to keep the airport operational despite hazardous conditions. However, the legal proceedings were protracted and complex, and eventually, all defendants were acquitted due to insufficient evidence and procedural shortcomings.
