‘Scrubs’ Trailer Proves How Vanessa Bayer’s New Character Will Handle the Todd

The Scrubs revival is an almost complete reunion of the original cast, although it has been trickier to bring certain characters back into the story than others. Certain figures, like Neil Flynn’s Janitor and Ken Jenkins’ Dr. Kelso, still haven’t been announced as returning for the sitcom’s new era. Thankfully, those two appear to be outliers for now and are expected to show up at some point either way. While it would arguably have been pretty easy to have them both return to Scrubs, the writers clearly wanted to make things more challenging for themselves by opting to bring back someone who is far more difficult to write for.

Rob Maschio played the Todd in all nine seasons of Scrubs that aired between 2001 and 2010. Although not typically an actor, Maschio’s aptitude for one-liners made him a highly quotable fan favorite. That said, Todd’s signature brand of inappropriate workplace banter just wouldn’t fly in 2026. Although Scrubs has always taken reality with a pinch of salt, bringing the Todd back in his original form was probably a step too far. Thankfully, the revival has found a brilliant way for the innuendo-heavy surgeon to play a part in the upcoming run. All it took was the creation of a brand-new character.

The Scrubs revival recently released its first proper trailer after months of brief teasers. Not only is it great seeing the more familiar faces back in action, but it’s also cool to see how the show’s new characters will slot into the Sacred Heart dynamic after more than 15 years away. One of the most notable introductions to the cast is former Saturday Night Live star Vanessa Bayer, who is playing a new character called Sibby in the Scrubs revival. As the leader of Sacred Heart’s wellness program, one of her biggest responsibilities is to monitor the conduct of everyone at the hospital.

This is proven by how Sibby interacts with John C. McGinley’s Dr. Cox, whose brash teaching style appears to have been locked in a time capsule and has remained unchanged. While berating one of the new younger doctors, the trailer shows Sibby appearing from nowhere and warning McGinley’s character to “bring it down.” When Dr. Cox asks, “Don’t I get three strikes?” his response comes in the form of: “You have 900 strikes.” The humorous moment paints an interesting picture of what the dynamic is like between the two characters. Dr. Cox seems to know the updated rules/guidelines, but he doesn’t have any real interest in adhering to them. Classic Cox.

The recent increase in mental health awareness, especially in the workplace, makes the addition of Sibby to the Scrubs revival more than logical. It means Dr. Cox can still be his same old self, but the sitcom is also acknowledging that parts of his attitude are an artifact of their time. In reality, Dr. Cox would probably be punished or even let go after so many warnings from a hospital official, but Scrubs being a sitcom, there’s more leniency than there would be in a more realistic setting. As useful as Sibby appears to be when dancing around the issue of Dr. Cox’s harsh teaching style, she’ll be even more vital to the Todd’s Scrubs comeback.

Todd was always the most inappropriate Scrubs character whenever he was in a scene. Although his penchant for a good high-five has aged without issue, Todd’s love of making almost everything into a dirty joke or sexual reference is deemed more problematic in 2026. While some of his more minor moments in this respect would probably be okay, ABC will be looking to crack down on Todd’s filthy outbursts. That’s where Sibby can come in. Just as she was getting on Cox’s back about how he spoke to his staff, I’m sure Sibby will always be close by whenever Todd is gearing up to say something that could make people uncomfortable – and that’s pretty much all the time.

Maschio clearly has fond memories of playing the Todd. Since the show ended in 2010, he has consistently referenced his time on the show, especially in his prolific Cameo videos. He’s also been incredibly vocal about how happy he is to be back for the revival, and given how popular his character was, it makes perfect sense that he’s once more donning his iconic flaming surgical cap. If Sibby is going to be keeping a watchful eye on the Todd’s behavior, which I think is likely, then introducing her is a masterstroke. However, it does beg the question of just how involved Maschio can and will be in the Scrubs revival.

In January 2025, Maschio implied he was wondering the same thing. Posting to his Facebook page, Maschio spoke warmly about how he has bounced back from the dark period brought about by the COVID-19 pandemic, and names his Scrubs return as an added bonus. That said, he does qualify the statement with: “I don’t know how much I will actually be in the Scrubs revival.” At the time, I was a little confused. What was the point in recalling him to the cast if he was barely going to feature? Now, I think I’ve figured it out.

What I think is going to happen is that Sibby will pretty much always be in Todd’s vicinity, hiding out of sight until she’s needed. When she senses something about to leave Todd’s mouth that she knows is going to be inappropriate, she’ll make a surprise entrance and cut him off. With so much of Todd’s dialogue in the show’s original era consisting of rude words and sexual imagery, it would make sense if he barely had any lines that weren’t censored by Sibby. This explains why Maschio doubted how heavily he’d feature in the final edits of the upcoming Scrubs episodes – maybe there just wasn’t much of any substance that could be used.
